Job Title: Site Nurse-LPN

Position Summary: The site nurse is responsible for the ongoing, overall healthcare service delivery, coordination and monitoring of all assigned individuals.
Essential Functions:
- Completes regular site visits to all assigned sites to make formal and informal observations of staff performance, individual health needs, ensures implementation of changes in plans of care.
- Responsible for review and documenting the ongoing health needs of assigned individuals and works with all members of the individual’s team to formulate, update and document associated plans of care, nursing goals and objectives and interventions.
- Participates in conferences and other meetings with individuals, parents, guardians and/or staff regarding individual progress. Provides input and recommendations during the planning process of each individual regarding medical, physical and emotional status, progress, needs and desired outcomes. This may include attending medical appointments and/or coordinating ancillary support services for assigned individuals (i.e. PT/OT/SLP/Dietary).
- Oversees medication administration and health related activities ensuring compliance with state and federal rules and regulations. Provides individual specific training to staff at assigned day programs and ICF/IDD sites. Completes quality assurance audits and regular medication pass observations at assigned sites.
- Adheres to assigned work and on call schedules at assigned sites. May be asked to work other shifts, weekends & holidays. May be asked to cover other nurse’s assigned sites. May be asked to assist with additional sites (i.e. camp).
- Assists in new and annual training of staff in the areas of OSHA and Blood Borne Pathogens, Medication Administration Certification 1, 2 & 3, Annual Medication Administration Refresher & Return Demonstrations, individual specific training and other topics as determined.
- Represents The Society with a professional attitude and positive intention while promoting effective internal and external relationships, communication and advocacy. Serves as a self-motivated, dependable, flexible and collaborative leader.
- Adheres to The Society’s policies and acts as a role model in adherence to these policies.
- Performs all other duties as assigned.
Bona-fide Occupationally Required Competencies and Credentials:
- Licensed in the state of Ohio, LPN with pharmacology or RN license. At least one (1) year of experience in the field of developmental disabilities.
- Must obtain and maintain certification in CPR and PMT as required.
- Displays knowledge of Ohio Department of Job and Family Services, Ohio Department of Medicaid, Ohio Department of Health and Ohio Department of Developmental Disabilities and Center for Medicaid and Medicare regulations.
- Must communicate effectively both verbally and in writing.
- Must have basic computer and word processing skills.
- Must be able to follow verbal and written instructions.
- Must have a valid Ohio Driver’s License, good driving record and carry liability insurance coverage limits required by The Society.
